Access to the NHS is not determined by nationality, it is decided by residency & whether or not you have contributed to the system. Contrary to popular belief, there is no such thing as free healthcare, it is funded by tax payers.

My advice is to contact the Department of Health and Social Security. In order for an NHS card or number to be assigned, there first must be a need. You may have to move to England first, then contact the DHSS to receive your NIN (national insurance number) If you are not resident in the UK then you do not have a need for such number or card.http://www.dhsspsni.gov.uk/index.htm
